PCGS requires the coin to be MS to get the FS designation but NGC does not. NGC will give any coin the FS designation regardless of grade.

I think that is pretty close to all of them unless you want to include all 6 reverse types from 1939. All 3 mints used both reverse of 38 and reverse of 40 dies. There are also the satin finish coins from the uncirculated sets from 2005-2010 that are different from the regular circulation strikes.

Yes. The 1997 coin is a bit more scarce than the 1994.

Yes. It is considered a special finish coin only available in that set. Similar to the one from 1997 that has the same finish.

Most likely some type of debris trapped inside the capsule. The US mint is notorious for bad quality control. My Peace dollar came with some black fibers inside the capsule.

Although this question would be better as a separate topic, there are two reverse styles for 1939 Jefferson nickels from all three mints. The "Reverse of 1938" had been used on all 1938 issues. The "Reverse of 1940" was introduced at some point in 1939 to strengthen the reverse details and is most noticeable on the steps. (The Reverse of 1938 was also used on a small minority of 1940 proofs but not on circulation strikes.) It is difficult to distinguish the differences on coins below Extremely Fine condition, so these varieties are generally collected on higher grade (mostly uncirculated) coins.
