2012-P Quarter Hawaii deterioration die

Hi everybody, need some help. Did the die not properly line up on this coin or shifted somehow. You can see it in GOD we trust and at the bottom in quarter dollar. Thought I would share and get other thoughts on this. Thanks

Definitely a worn die on the obverse. You can see the abraded areas around Washington. What you are seeing in the lettering is probably partially die erosion and some strike/mechanical doubling as well. 
 

Die erosion doubling is like it sounds. The dies erode and allow metal to flow outside of the intended design slightly. 
 

Mechanical/Strike doubling is caused by the die and/or planchet shifting in relation to each other during the strike. It “smears” the design slightly to simplify it.

From the stuff I have been finding like yours Hinkle where it is only in areas they have been polishing the dies . Woods has it ;)
