Is there an online form that I can print for NCS /NGC submissions?

I have a few coins to submit and I am out of forms. Can anyone here direct me to a printable online form? Thanks

They don't exist. I think it has been suggested by someone here ( ) at least once or 500 times, but as of now it doesn't exist.

What's funny is that you can do PMG online.

The good news is that if you order forms, they'll be there very quick. I faxed in my order on Monday, I believe, and I just got a big priority mail box full of them today.
