This 1886 GB 6 pence is a very inexpensive coin graded AU58 . I purchased from Heritage Auctions, upon removing their label from the reverse I saw internal damage to the NGC holographic. The picture on NGC does not have similar damage. How does this occur?

   These holograms, which are placed on top of the plastic on an NGC holder, are frequently damaged by the sticky labels that dealers and auction houses place on them, especially when the label is left on for a long time.  Yours looks like someone tried to remove residual "gunk" from such a label with a knife or other hard object, resulting in parts of the hologram being scratched off.

I can’t feel the hologram, or the damage  on the exterior of the slab.

The slab may have changed hands many times in the past before it made it to Heritage Auctions. It is possible there were other stickers affixed to it previously and someone attempted to remove a previous sticker but did so unsuccessfully as they damaged the hologram. The hologram is not on the inside of the slab and the slab itself seems to be intact in the photos you provided.
