"Return Mint" needed for GSA hoard submission?

Hello, I am brand new to NGC, and am about to make my first submission, which is a 1884-CC Morgan dollar in the original GSA holder. 

I have a few questions:

1: Do I need to check the "return mint packaging" and pay the extra $5.00 fee? 

2: I assume (hope) that the coin is graded in the original holder without removal, and returned in the original holder?

3: Do I need to include the outer box, and certificate, or just the coin in its holder.? I also have the original invoice from GSA from 1980 if needed.

4: I am paying by Visa credit card, and noticed on the form that the 3 digit "security code" on the back of the credit card is not asked for.   Do I need to provide it anyway?

5: So, for shipping to NGC, do I simply need to include the coin in its holder (and box, COA, and GSA invoice?), and  the filled out submission form?  This is my first submission, and wanted to double-check on the proper procedure.

Thanks for hopefully answering my newbie questions.  I have attached a composite image of both sides of the coin in the holder. These were taken on a flatbed scanner at 2400 DPI.

For the checkbox 'Return Mint Packaging' - I can see that any time I downloaded and printed the form, on one the pages have this referenced that reads: capsules, COA and mint boxes. If you are sending any of these, you can select that checkbox.

Hello,

Please do not send the box or the COA's when submitting GSA's.  We only want the coin in the plastic.
