Calling on IHC experts again

Coolness factor aside, how common are clashes on Proof IHC? This is an 1889 owned by Toneddollars with clashes on both obverse and reverse. On the reverse I at first thought that the line running through the C was a die crack, but it seems to match up with the clashing shown at the left of the O.

They are from a camera. I was just surprised that since so much care is taken when producing proofs that this would happen. It wasn't like they made millions of them and couldn't check each one.

Die clash is common on currency strikes but fairly scarce on proofs. The Mint did get a bit careless with its proofs during the 1870s and 80s.
