How did this happen and what's the proper term for this type of an error ?

VAMs are die varieties on Morgan and Peace dollars, the term is not used on any other series. What you have there are mint errors, not die varieties. The first one appears to be a lamination, and the second one appears to be, as you mentioned, die chips.
