Completely blank presidential dollar

While searching through a bank box of 'P' marked Adams presidential dollars, I found one of the coins was a completely blank planchet (no obverse, reverse or edge lettering). I have two questions. How rare is this occurrence with the presidential dollars and will NGC grade such a coin?
