ReHolder Due to Mistaken Identity

I have a modern commemorative that's apparently mis-labeled. I was instructed to send it back to NGC for re-labeling. Anyone know the process to follow and/or form to complete? TIA.

I use the online forms and goes as follows

 

Regular form

List the coin

No Grading service

Reholder service

Detail the problem in the instructions section

This should be NO cost to you except for shipping and insurance.
