Plate Coin Submission Question

If I wish to submit a coin that is a plate coin from a "bible" for a series, how do I indicate that on an NGC submission form? It isn't obvious to me how to request it on a submission form. Is it a variety designation or a "customer notes" thing? I appreciate any help.

I would think you would have to submit some form of documentation to support your conclusion such as a bill of sale attesting to the coin's provenance or a signed notary statement from the book publisher -----maybe more if the coin's chain of ownership is still in question. Good Luck.
