Multi-Holder

If I send in one coin for crossover and 4 coins already graded by NGC to be put into a multi holder is the holder free as indicated for ungraded coins or should i include $20 for the reholdering of the 4 coins? Also if I need to include the $20 how would I go about it as the reholders would need to be one invoice and the crossover on another and I want all five coins put into one multi holder?
