Submitting after conservation

If I check the box to "Submit all coins for grading":

 

Do I need to send a completed NGC submission form with my coins in addition to the NCS?

 

If not what grading service level will be used at NGC? Can I specify what grading level I want used?

If you mark the box to have your coins graded after conservation, you do not need to also fill out an NGC form. We will complete that paperwork for you for only those coins that do grade with NGC. We will select a tier for you that is the most economical based on your coins' value and type. You can certainly request a specific tier.

I have a related question:

 

Lets say I submit 5 coins to you for conservation and to be sent on to grading.

4 of the coins get conserved by you, one of them ends up in a details slab (NCS)

 

Will I automatically not be eligible for the NGC "economy" ($16) service and be bumped into "EarlyBird" and have to pay double ($30 each coin) for the grading?
