How to specify NCS conservation on online form? 1 1

I am preparing to submit a coin for conservation for the first time and I do not see an option to indicate conservation on the online form. 

I note that the fillable PDF offers conservation options, but I have had problems in the past with the fillable form and now always use the online form.

Can I use the online form to submit a coin for conservation?

Hello @3sidedCoin, thank you for contacting us. Unfortunately, you will need to fill out the separate NCS PDF form in order to include that service with your coin if you are using the online form. The online form does not have a way to include that service at this time, so the additional PDF form is necessary. We are sorry for any inconvenience this may cause but thank you for checking!
