The 0 took a hit which displaced the metal. I cast my vote for damage.

For the situation you present, the dies would have had to have been an overdate with a 0 over 0 which would have produced hundreds if not thousands of the same cents which 54 years later would have surely been a discovered variety as it would not be a subtle one to be detected. This did not happen at the Mint on this modern of a cent.
