   The raised, irregular areas on the obverse of this well-worn 1961-D nickel are likely metal solder or other foreign matter that someone placed on the coin.  It appears that the coin was once mounted on (attached to) another object.

Not an error of any kind. It is just damage, exactly what caused it doesnt really matter. Damage is damage4.
