Is this a striking error? 0

My son found this by the water heater while I was checking the pilot light. Would this be a error or not? There are 2 "pin" holes that cover up what i believe is 1969. You can also see them kind of coming through the reverse. The other hole im not shure about. Just curious if it is or not. Thank you in advance!

Found by the water heater makes me think it was used to level something, and the damage came from that.
