Welcome to the Forum-

 It's damage. Looks like the reverse of a wheat cent has been pressed either intentionally or unintentionally onto the Obverse of your coin. This is sometimes referred to as a " Vise Job" as it is not unusual for someone to squeeze two coins together creating a reverse image of one coin on the other

Yep, that's the O in ONE and top of the E in CENT, I believe. Vise squad got hold of that one.
