Hi, I am submitting my first coin to NGC, the 2019-S ERP Silver Eagle. On the submission form packet, there is a form "Consent to Remove Coin(s) from Holder (s)". Am I suppose to complete and sign that form, if my coin is still in the sealed US Mint package?

Also, should I write the Form # and/or Account # on the US Mint package shipping box?

Just want to make sure I complete everything correctly. Thanks!

Is my understanding the consent form is only if it is already encapsulated by another grading company.

On the shipping box you should just write the Service you using (Express/Modern).  I've always taped the Invoice/Line number on the mint box inside the package with no issues.
