Some dumb questions about submitting to NCS/NGC...

I have to work all day tomorrow and won't be able to call customer service.

Planning on submitting 3 coins to NCS - 2 cents and a $5 gold.

So, based on what I've read, these need to be on two submission forms because gold has to go on one, and the others will need to go on another?

What happens after NCS?  Do I need to also submit an NGC submission form, or is it assumed that they would be transferred to NGC for grading?  If I need separate submission forms, do I then need to submit one for the gold, and one for the cents? Or, by filling out both box 5 and 6, everything is good to go (would still need 2 total submission forms and not 4)?

If 2 submission forms, I have to pay for return shipping twice?

Thanks for your help!

I believe you only have to submit the NCS form. My question is what exactly are you having conserved and why? What grades do you expect to receive?
