The ( B) in liberty and the 9 seem to be flawed.. Is it an error? If so what kind?   And when u flip the nickel over it is not correct it is upside down on the back. I'm not sure how to say it now for some reason.. Maybe not stamped properly..

Looks like the "B" and the "9" might have some added metal as the result of small die chips. The Indian Head silver round was evidently struck with what is known as "medal turn" orientation - both sides oriented the same, as opposed to "coin turn", which is a 180 difference in the orientation of the two sides.
