I have seen damaged coins in Mint Sets before but they were damaged somehow after they were ejected from the striking chamber and before they were packaged so they are still classed as PMD. Sometimes they get caught in the crimping machine.      I can see why you would question it though as it is unusual.

The damage on the dime comes from the edge of the coin being run over by the heat sealer that seals the coins into the cet.  The 1899 quarter has not been is such similar holder.(They didn't have mint sets back then or heat sealers)
