Just damage. It didn't leave the mint that way. Looks like it has been punched with something. You can see the displaced metal.

I agree with Greenstang. Whatever caused the hole in the rim also caused the arrow-shaped bit next to the mint mark. There is no way to determine the exact cause of damage to a particular coin, in most cases.They tend to lead rough lives sometimes.
