I'm thinking heavy post minting damage on that one.  I'd say its been scraped up somehow....maybe it was dropped in a parking lot and got run over a few times then someone picked it up and put it back into circulation before it was damaged further.

Definitely PMD. There's no way that could happen during the striking of the coin.
