Picture would be helpful, but usually this is the result of overpolishing of the die.  it is also possible for the recesses in the die that create the initials to become clogged with "grease".  "Grease" is a catchall term for lubricating oil that has picked up a bunch of dust, dirt, metal filings, other small particulate matter or just general fine debris.
