limited edition - which label

I have a set of 6 crowns which are limited editions of  only 25 sets.   the certificate of authenticity states 15/25.   which label do i requestt on the NGC submission form?

Thank you for your question. Can you please provide me with additional information regarding the coins? The date, denomination, and if the coins are minted as a set, please provide the set name/designation.
