submission forms and cert.#s

hello, just wondering if there is a way i can get a list of my old submission forms. i need the cert.# of a old coin that i had graded that was taken out the holder by mistake ( not knowing better ) to have a vp added to it . when it was sent in it was graded x-f , when it came back it was graded vf.
