I do not know the answer for sure but I expect that you would need to send in the additional coins on a seperate submission form. You can send both submissions in the same package, and unlike ats you can elect to have both submissions returned in the same package thus saving on return postage.

coinbuf is correct. You should submit only the five coins used with the coupon on the form. You can include other submissions in the shipment to NGC and you can write "Ship with" and list the invoice numbers on one of the forms so that they all get shipped back to you (and save you postage).
