How to fill out submission form???

I've got a box of ASE 25th Sets that I want to submit to NGC.

Looking at Section 8 of the form, it asks sender to fill in one line for each coin submitted.

Do I need to list each coin in each set separately or can I just use 1 line for each set?

I THINK YOU SHOULD CALL NGC,JUST SO YOU DONT MAKE ANY MISTAKES.I AM ALSO UNSURE OF WHAT TO DO SO I AM GOING TO CALL IN THE MORNING.THESE ARE TO PRECIOUS TO MAKE ANY MISTAKES.
