NCS Coin Grading

Does NCS grade a altered coin as a damaged coin? For instance, I have a Ike dollar that is clearly smaller in size than a normal dollar. NCS graded it dam rim. How can that be? If the planchet is not smaller by foreign design and someone cut the planchet down, placed a reed back on it, would it not be altered? Could NCS have gotten this wrong?
