Submitting coins for deluxe photo proof

I won the 2008 "most creative" signature set award and want to submit 5 coins from my winning set for deluxe photo proofing using my 5 free coupon. I have several questions regarding this though.

 

1. What submission form do I use and how do I fill it out?

 

2. I have foreign and US coins I want to submit, can these be combined?

 

3. I noticed on the web site that US commemoratives through 1954 are eligible for the deluxe level, what about modern commemoratives?

Photo Proof has its own submission forms, and these are obtainable by calling Customer Service.

 

We have full Photo Proof services for all USA coins, but only the Passport service is available for most world coins. The sole exceptions are Britain, Canada, USA/Philippines 1903-45 and South African gold 1892-1900.
