Designations

When did the TPGs start using descriptive designations? What dates, as specific as you can remember? In particular, I am wondering about FBL and Cameo. I think PCGS started FBL in 1997, but does anyone know a more specific date, and when did NGC start? As a tangent, when did they start FB, FT, FS, FH, RD, BN, *, and any other designations?

I think the NGC star designation was very recent, comparatively. I just went through and looked at NGC's news articles, and they expanded the star designation to all series' on 6/18/2002.

for pre 1915 proof coins and for 1936-42 proof coins the cameo and ultra cameo designation came into play in mid 1998
