Thought this was pretty interesting and wanted to share it with others who may feel the same:

Looks like they ran it through the Castaining machine all the way, but the coin didn't drop out at the end of travel so when they cranked it back to the starting point the coin got pulled back through receiving a second impression just slightly offset from the first one. (If the coin had simply made two regular trips through the machine it is highly unlikely that both impressions would be the same orientation and so closely aligned to the original pass.)
