Need submission form 0

I have called customer service 3 times in the past few weeks to request a coin submission form . Still no coin submission form. What gives?

Unfortunately, we are currently out of stock of our paper submission forms. If you have requested more paper submission forms, do not worry they will be fulfilled once we get more in stock. If you need to fill out a form and cannot wait for paper forms to be sent, I recommend going to our website and downloading our PDF forms to print out. If there is anything else we can help you with please let us know!
