Can someone tell me what happened to his shoulder Is that a die chip?

Pictures are a little out of focus but it looks like a die chip which is not considered an error.

I am uncertain from the photos if that is raised or incuse (impressed into the surface of the coin).   If it is raised then as GS said most likely a die chip, if incuse then it could be a strikethrough which would be considered an error.

It's difficult to say from the pics but I'd lean towards die chips as it looks like three separate imperfections (I'm looking at the third image in the OP, definitely looks like maybe two?). Whether die chip/s or a strike-through the value is minimal,  although the result is interesting. A keeper either way in my book.
