The fee calculator function in the PDF submission form does not provide for the additional fees (+$5) and (+$10) for the Add/Holder COA or editing "Total Due". Do I just add $15 to the total in post/alter the doc? Disregard and add $15 to the check? Also, is this where the add-ons should be noted? Thanks in advanced.

I added the $15 to my total even though the PDF wouldn't calculate it. hopefully that isn't a problem. I hand wrote in the customer notes just as the NGC website stated to do so.
