first ngc send

Dear NGC,

Good morning, I am your new member and I do not really manage :). I am asking for advice. 


I have filled in an NGC form with shipping forms and a table with information about the coin, only one for a test.
I have also marked the NCS service.

Do I have to send the zipped coins once with the completed form to the following address, im from Poland:

i will send by UPS spedition.
I am asking for confirmation and I will send you a parcel
