Anyone have a submission form?

Call me cheap,but I don't want to renew my membership at this time but I need a submission form for 2 coins for a designation review.Membership ran out two weeks ago.I asked NGC how to get more an they gave me some bogus or outdated instructions how to get them from Ebay. Anyone have an extra? PM me please.Thanks
