George Washington's Brain

I have a 1986P Washington Quarter with what I believe is a serious planchet flaw. Where Washington's hair should be is instead a mess lacking any detail, looking more like the hair, scalp and skull have been removed to reveal the brain beneath. The outer parts of both fields also has the same appearance; letters and numerals are present and legible under magnification but show numerous places where metal has been broken out or flaked off (before the coin was struck?).

The edge shows some reeding but again there are spots where metal has broken off. There also appears to be a small clip at roughly 8 o'clock.

 

Can someone suggest an error site that has pictures and descriptions of planchet errors?
