Unable to Print Submissions Form

Hi NGC Forum,

I just created an order but when I go to print the forms, the website is displaying a blank page and no printer pop-up appears.  Anyone else having this issue or know how to solve it?  I checked pop-up blockers already.

FYI - I've created many submissions in the past.  Never had any issue with printing.  I also tried several browsers (Safari, Chrome and IE).

I can print labels with no issues, just not the submission form.
