1964 mint on back beside monument also has 2 impressions on obverse side

Welcome to the forum. Your nickel has its mint mark in the normal place for a 1964 and earlier Jefferson, except for the years of WWII, when the mint marks were placed  above Monticello, to signify a change in composition. The mint marks were absent (by design) in 1965, '66, and '67,  and then returned, on the obverse below the date, in 1968.

The obverse of your coin has been damaged by a punch, nail, or some other object. It is not a mint error.
